If you are a veterinary surgeon, and would like to refer a case to us, please print, complete and fax the form below. Following completion of the case at our clinic, owners will receive any aftercare advice and medication instructions. They will also be advised as to the need for further examinations by ourselves or the referring vet. We will both fax and post the referring vet a letter explaining outlining the work done on the case and our expectations for the future.
